  Rolls and Responsibilities:-
  
  1. Meet with patients and family members to discuss needs, conditions, resources and options. 
  
  2. Work with patients to set goals, form plans to reach those goals and monitor progress. 
  
  3. Coordinate care plans and support for patients who require multiple services at a time. 
  
  4. Provide informal counseling and advice on how to live with illness or injuries.
