    Job Description:

    Our medical billing division's "Data Integrity Anchor" and "Initial Claim Velocity Engine" is the Charge Entry Executive (Healthcare RCM Data Capture & Claims Initiation Specialist). Your main responsibility is to administer, audit, and carry out the initial data entry transition of healthcare encounters, converting raw clinician encounter forms, doctor visit super-bills, electronic health record (EHR) clinical notes, and patient intake logs into clean, fully coded billing entries and electronic claims prepared for clearinghouse submission, while downstream teams handle accounts receivable and payment reconciliation.

     

    Key Duties

    Auditing and Preparing Encounter Batches: Obtain, classify, and record daily patient encounter batches from various healthcare facilities, ensuring that all clinical super-bills have corresponding provider documentation prior to input.

    Verify patient demographics by cross-referencing patient data fields in the practice management system, such as full legal names, dates of birth, active policy subscriber numbers, and coordination of benefits (COB) rankings.

    Procedural and Diagnostic Entry: Using the clinician's structured encounter sheet as a guide, accurately enter alphanumeric codes for clinical procedures (CPT), medical equipment tracking codes (HCPCS), and diagnostic classifications (ICD-10-CM).

    Modifier Assignment & Validation: To properly describe unique treatment scenarios to insurance systems, use conventional billing modifiers.

    Provider Information Mapping: Ensure that the required National Provider Identifier (NPI) codes and taxonomy classes map appropriately by connecting charge lines to accurate rendering, referring, and billing provider profile entries.

    System Claim Scrubbing & Pre-Submission Auditing: Before sending files to the clearinghouse, run data inputs via software scrubbers and system billing rules, promptly resolving code incompatibilities, missing values, or unbundled items.

    Missing Encounter Investigation: Keep an organized record of any incomplete physician documentation and forward any missing information files to the clinical or administrative teams of the healthcare facility for prompt cleanup.
