• Charge Entry Executive Charge & Billing Operations

  Job Description

  Duties:

  Review patient superbills, charge tickets, operational encounter forms, and clinical notes for accuracy and completeness prior to entry.

  Key diagnostic codes (ICD-10-CM), procedural codes (CPT/HCPCS), service units, charge amounts, rendering provider details, and facility locations into the billing system.

  Apply appropriate procedural modifiers based on coding rules and payer-specific guidelines.

  Verify charge-to-demographic linkage, insurance eligibility, and prior authorization requirements before final charge release.

  Identify, resolve, and report charge capture discrepancies, missing charges, unbilled encounters, and front-end claim edits (clearinghouse rejections).

  Maintain daily volume and accuracy productivity metrics while ensuring clean claim submission to clearinghouses and payers.

  Adhere strictly to Health Insurance Portability and Accountability Act (HIPAA) privacy guidelines and internal billing compliance standards

   

  Focus Skills:

  Thorough understanding of US Healthcare Revenue Cycle Management (RCM), medical charge entry workflows, and charge structure principles.

  Familiarity with CPT, ICD-10-CM, HCPCS coding frameworks, modifier usage, and National Correct Coding Initiative (NCCI) edits.

  Excellent data entry speed and numeric keypad accuracy.

  Practical experience with medical billing and EHR/EMR platforms (e.g., eClinicalWorks, AdvancedMD, Kareo, Epic, NextGen, Kareo).

  Strong attention to detail, analytical mindset, and ability to identify missing charges or documentation gaps
