  Who you are:  

  To precisely insert clinical charge codes, service dates, units, and fee schedules into billing systems, review patient superbills, charge tickets, operational schedules, and electronic medical records (EMR). Before sending a claim, check CPT, HCPCS, ICD-10-CM codes, and modifiers against medical records, NCCI revisions, and payor coverage criteria to remove coding errors. To avoid registration-related claim denials, cross-check patient demographics, insurance coverage information, policy numbers, rendering/billing provider NPIs, and pre-authorizations. To maintain high Clean Claim Rates, review claim scrubber rules, fix front-end claim modifications, fix missing information, and flag missing charge documentation. Keep operational tracking spreadsheets for pre-billing error trends, daily charge volumes, dollar values, entry turnaround times, and coding query logs.
